Enter the invoice number shown in the Online Payment box (6 digits for new invoices, 16 digits for older invoices).
Enter the patient's last name as shown on the invoice.

Please note that a 1.32% credit card transaction fee plus 20 cents surcharge will be automatically added to your account. AMEX and Diners are not supported payment methods.
